Teddy’s migraines, too, resulted from withdrawal symptoms from his medication, which Cawley and the other doctors had ceased to give him to allow him to live out his fantasy.

However, when confronted with the harsh reality of his situation and his tragic past, Andrew collapses and awakens in the hospital with Cawley by his side. While Andrew appears willing to accept the truth, Cawley reveals that he and the other doctors had previously helped Andrew achieve a similar state of tranquility, only for him to regress once again. Cawley warns that if Andrew reverts again, they will have no choice but to resort to a lobotomy.

The shocking revelation of the truth behind Shutter Island’s elaborate facade and the dire consequences that await Andrew if he cannot overcome his delusions leave both Teddy and the audience questioning the ethics of the hospital staff and the lengths they are willing to go to protect their patients.
